MidMichigan Health and its Affiliates are pleased to offer our patients online computer access to portions of their medical record through the MyMidMichigan web portal. If you would like access to this service, you must complete an online request and agree to these terms and conditions.

Release of Information: Past Medical and Prescription History

I authorize MidMichigan Health and all of its Entities and Affiliates and/or my provider to obtain my past medical and prescription drug records to aid in my care and treatment. This authorization may be withdrawn at any time, and if withdrawn, shall not have any effect on information disclosure made prior to the time of withdrawal, however, will not affect any disclosures that MidMichigan Health and all of its Entities and Affiliates are permitted to make under Federal or State law.

MyMidMichigan is not for Emergencies

MyMidMichigan allows you to access your medical records and to communicate with MidMichigan Health and Affiliates about non-urgent matters. You should NEVER use MyMidMichigan for urgent health issues. If you are having a medical emergency, go to the nearest emergency room or dial 9-1-1.

MyMidMichigan User ID and Password

Accessing and using MyMidMichigan is voluntary. If you choose to use MyMidMichigan, MidMichigan Health and Affiliates will assign you an activation code. You will need to create a unique User ID and a Password. It is your responsibility to protect your User ID and Password. Anyone with access to your User ID and Password may access your personal health information. It is your responsibility to change your password if you believe it has been compromised or shared with someone else. You should change your password at least every 180 days. Each time you log into MyMidMichigan, the system will retain an audit trail of your access.

Secure Messaging

If you send a request or message through MyMidMichigan, you can expect to receive a response within two business days. If your provider is not available when you send your message, we may route your message to a different provider. If you do not receive a response to your request within two business days, please contact your provider’s office by telephone.

MidMichigan Health and Affiliates providers or staff may send messages to you through MyMidMichigan. The system may send you email or text notifications to tell you that you have a new MyMidMichigan message in your account. For your privacy, these notifications will not include personal health information, so you will need to login to view the details of your message.

You may not use MyMidMichigan to request information or advice about someone else’s medical condition. MyMidMichigan messages will become a permanent part of the patient's medical record. Therefore, it is important that all messages sent or received from a patient's MyMidMichigan account are only about the patient.

Prohibited Content

The following types of content are prohibited and may not be included in secure messages or uploaded to your MyMidMichigan account. You understand that you are responsible for the content that you post and that MidMichigan and its Affiliates are not responsible for any legal issues that may arise out of your communications and activities on MyMidMichigan.

• Language or content that contains profanities, obscenities, or indecent or pornographic material

• Language or content that is abusive, harassing, threatening, or defamatory

• Illegal content or content that violates individual property, privacy, or publicity rights

• Personal attacks on an individual or group

• Discriminatory language or content including, but not limited to, discrimination based on race, age, ethnicity, religion, gender, physical or mental disability, sexual identity, gender identity, or socioeconomic status

• Photos or videos of individuals other than the patient


Federal and State law govern the confidentiality of medical information. MidMichigan Health and its Affiliates are dedicated to keeping your medical information confidential. When you access your information, you must share the responsibility for keeping the information confidential. Please see MidMichigan Health Notice of Privacy Practices

You also understand and agree that MidMichigan Health and its Affiliates may disclose your communications or activities on MyMidMichigan if necessary to comply with the law or to protect other people’s rights, including the rights of our patients, providers and staff.

MidMichigan Health or Affiliates Right to Terminate or Restrict Access to MyMidMichigan

If MidMichigan Health or Affiliates discover that you have shared your MyMidMichigan access information or that you have misused your MyMidMichigan privileges, we may suspend, restrict or terminate your participation in MyMidMichigan. MidMichigan Health and its Affiliates shall not be liable for damages arising from a breach of confidentiality resulting from your sharing or losing your MyMidMichigan access information.

MyMidMichigan Availability

There may be times when the MyMidMichigan system is not available to you. For example, system maintenance, back-ups or failures may limit MyMidMichigan availability. We provide access on an “as available” basis and you are not guaranteed access at any particular time.

MyMidMichigan Security

Encryption: We use 128 bit Secure Socket layer to encrypt the communication from your web browser to our secure web server. Firewall: We store personal health information only in the secure electronic medical record, protected from the Internet by a firewall. Dedicated web server for MyMidMichigan: We separate the web server and electronic medical record machines, and we do not save any personal information on the web server. This protects your information in the unlikely event of an attack on the web server. No caching on the client side: We disable caching of data in the browser and force the web pages to expire immediately. This prevents a follow-on user from viewing pages that you had previously viewed while using MyMidMichigan. Session timeout: After a period of inactivity, the logon session will time out. Monitoring: We constantly monitor the web server for evidence of unauthorized break-in attempts. We apply the latest security patches, as they become available.

Third-Party Links and Services

You may choose to use third-party services, such as translation or location services, with your MyMidMichigan account. For your convenience, MyMidMichigan may also provide links or references to other sites. MidMichigan Health and its Affiliates are not responsible for the content of these sites or services and shall not be liable for any damages or injury arising from using them. You may need to submit your information to a third party in order to use these sites. This could result in that third party using or storing a copy of your information. If you decide to use these services, you do so at your own risk and subject to their terms and conditions.

Third-Party Applications

If you as a patient find a third-party application that you would like to use to access and import your health information, MidMichigan and its Affiliates provide the ability for you to use this application. To do this, you must establish a MyMidMichigan patient portal account, since the user name and password you set for MyMidMichigan will be your authentication for any third-party application to access and import your health information. MidMichigan and its Affiliates reserve the right to disable any third-party application if needed.
